- Early involvement as our team participates on your hospital’s discharge planning team

- Home Environment Evaluation to assure specialized needs for life support equipment and safety are met
- Coordination of community support services including electrical, fire and EMT if needed
- Immediate post-discharge reassurance and support to home care staff and family
- Ongoing follow-up visits by a Respiratory Therapist to assess home care equipment
- Wide range of home care equipment available for purchase or rental
